Information You Have to Give

As part of your request you should include your present address as this will help us locate your information and this is where we will send our response.

Under normal circumstances we also need evidence to prove your identity and to confirm that you are entitled to receive the information - we require 2 pieces of proof of identity.  Acceptable evidence will be 2 of the following:

• Current passport
• Current driving licence
• Utility bill - less than 6 months old

Additional evidence will be required if you are unable to supply a passport or driving licence or if the request is being made by someone other than the patient.

You should also provide us with as much detail as you can about the information you want to access so we can locate it quickly. If we need to contact you for further information to help us find the personal data you requested you may have to wait longer for a response.

Once we have located your personal data we will post copies to you by Recorded Delivery unless you request otherwise.
