Our newly established Acute Frailty Service has been shortlisted in the Improving Care for Older People category at this year’s HSJ Patient Safety Awards, recognising their outstanding contribution to health and care for the Stockport population.
Following one of the toughest years in the health and social care sector, staff have continued to champion patient safety amidst challenges that the health and social care sector is facing.
The team has been shortlisted despite the tough competition from hundreds of applicants, based on their ambition, visionary spirit and the demonstrable positive impact that their project has had on patient and staff experiences.
The Acute Frailty Service aims to prevent avoidable hospital admissions and reduce length of stay in hospital for the most vulnerable people. It enables older people to be rapidly assessed, diagnosed, treated and supported in the most appropriate care setting. Where safe to do so, patients return home the same day (with support in place as needed).
Karen James OBE, Chief Executive, Stockport NHS Foundation Trust said, “We are delighted to have been shortlisted for the Improving Care for Older People award, recognising the collaborative efforts and dedication of our staff members over the last 12 months to successfully implement the Acute Frailty Service. We are committed to delivering improved outcomes for our patients, and to be chosen among the other incredible nominees is a wonderful achievement. This nomination has been a tremendous boost to our staff and I am sure it will bolster our continued efforts to improve our services.”
