About Our Service

Welcome to the Stockport Team for Adult Rehabilitation (STAR).  We are your local community neuro-rehabilitation team.

We provide multidisciplinary, co-ordinated rehabilitation and management for a wide range of neurological conditions such as brain Injury, stroke, multiple sclerosis, motor neurone disease and spinal cord injury.

Our specialist team includes occupational therapists, clinical psychologist, physiotherapists, speech and language therapist, and an assistant practitioner.

We provide rehabilitation in the most appropriate place for you. This may be in the home, at the gym, in hydrotherapy, in the workplace or as an outpatient at The Devonshire Centre for Neurorehabilitation.

Working with you we can offer a personal rehabilitation plan which is goal orientated to encourage you to reach your potential such as regaining fitness to be able to attend the gym following a stroke.

We give specialist advice and education to enable you to self-manage particular health issues: such as the MS fatigue management programme.

If you have a complex, deteriorating or life limiting condition our multidisciplinary team can help you and your family to manage this in a practical and timely way, such as assessment and training in the use of advanced communication technology.

You can learn, with our support, new ways of coping with emotions, such as anxiety or anger management.

You can benefit from advice around strategies to deal with memory and concentration problems.

Our Expertise

In our team are highly qualified professionals with specialist training in

  • Normal movement
  • Cognitive rehabilitation
  • Splinting
  • Swallowing management

Many of the team hold post graduate qualifications including an MSc in advanced practice. This means we have the expert multidisciplinary skills to deal with complex neurological conditions.

We work closely with consultants, specialist nurses, social services, the voluntary sector and other services. This means that you are offered the best advice available for managing your condition.

You said about us: “You went beyond doing your job and we could not have got this far without you” “You helped me enormously and kept me going at times when I have felt defeated”

How to Use the Service

Your consultant, GP, health or social care professional can refer you to the team. You have to be registered with a Stockport GP. We see people over 16 years and under 65 years of age. To be accepted for rehabilitation you must need to have therapy from at least two disciplines such as physiotherapy and speech and language therapy.

When we have received your referral usually two members of the team will visit you at home and carry out a ‘screening assessment’.  This involves using a standard assessment document where we ask you questions about your condition and how you manage activities during the day.

Following this visit the team will decide if you require rehabilitation from two or more members of the team. Depending on your situation and need you may be placed on a waiting list for each of the disciplines recommended.

It may be decided that you do not need rehabilitation from the team. We provide advice on the first visit and that is enough. If you only need therapy from one discipline we will try to refer you to another service. Whatever the outcome we will keep you informed throughout.

When you start receiving therapy from STAR, we will decide with you how the team will support you.  This often involves goal setting and working out a rehabilitation plan.
