About Our Service

The Community Advanced Clinical Practitioner (ACP) and Matron Team provide a holistic, home based service for patients with complex health needs who are at risk of hospital admission or, who have already experienced recent hospital admissions. They work with each individual and their carers/families to understand health and wellbeing needs and explore options for treatment and support. This might involve further tests, changes to medications or referral /liaison with other services to ensure a co-ordinated approach to treatment.

The ultimate aim is to improve well-being and prevent avoidable hospital admissions by planning for the future and promoting the right care taking place at the right time. Patients will be offered an Enhanced Case Management (ECM) plan to maintain wellbeing and actions agreed with the patient/carer to take if their health is worsening. Links may be offered to the Crisis (Urgent) Response Team who work alongside our service responding to urgent calls to prevent admission. 

Our Expertise

Advanced Clinical Practitioners are senior Nurses or Allied Health Professionals (such as Physiotherapists, Paramedics, Pharmacists) who have completed a Master’s degree to equip them with the expertise to undertake clinical assessment, interpret test results, diagnose illness and treat a range of conditions which includes prescribing medication. The Matrons are senior Nurses with additional training and non-medical prescribing qualifications who work independently, supported by the ACPs.

How to Use the Service

The service operates Monday-Friday 9am-5pm. Please see the referral criteria document below. Referrals can be made as follows:

  • Primary care- via Managed referrals on EMIS. We can be contacted to discuss cases via Single Point of Access on 0161 204 4777
  • Hospital Teams- using our referral form which is located in the “do it online” section of the Intranet
  • Community Teams- via interservice referrals using our referral form which is on EMIS community
