The Active Recovery Service provides free, short term physical, social and emotional support that will:
- help you avoid a stay in hospital, when you can be safely supported at home or in another appropriate community setting
- help you following a stay in hospital
This support will be provided for a period of time appropriate to your needs, and can include:
- rehabilitation services, to help you gain and keep more independence
- services focused on improving your health and well being
- support that can help you connect, or reconnect, with your local community and activities

The Active Recovery Service is delivered by an experienced team of professionals and specialists, including Nurses, Physiotherapists, Occupational Therapists, Social Workers and Support Workers.
Covering the entire Stockport Borough, the team works closely with other health and social care professionals in the local area, including your GP, to ensure you receive a service that meets your personal needs. |
When your service starts you wil be given the names and contact details of the team that will be personally supporting you.
Firstly, your team will help you decide what short term support is needed to help you recuperate. This will be outlined in a Support Plan, which will be created personally for you. |
