About Our Service

Welcome to the children`s continuing care team. We care for children who have complex health needs to allows families to have some respite.

Respite care is a short break to give you chance to spend some time looking after your own needs, or  quality time to spend with your family. Respite care can be adapted to meet your own individual needs.

 

This service is provided for children and young people with complex health needs aged between 0 – 18 (19 years if in full time education). Children and young people may be seen by the service if they have special needs and one or more of the following:

  • They require nursing and/or medical care
  • They require respite provision
  • They require continuing care assessment
  • They have been referred by Medical Consultants, Paediatricians,  GP’s, or Social Services.
Our Expertise

The Continuing Care Team is made up of Senior Children’s Nurses, Staff Nurses, support staff and admin support. The team provides accessible, flexible and high-quality specialist community paediatric care.

The team co-ordinates all aspects of the child’s needs from early support, care within the home, respite provision, through to transition to adult services. They may achieve this by:

  • Working in partnership with parents and carers, other healthcare professionals, education and social care staff and voluntary agencies
  • Assessing and promoting the needs for the child and family to facilitate good health
  • Collaborating with all services involved in the child’s well-being to plan care
  • Training and monitoring other care workers from statutory and voluntary agencies. Completing continuing care assessments
  • Providing respite care to support families.
How to Use the Service

Your medical practitioner or Social Worker will complete a Common Assessment Framework form (CAF). You will them have a Health Assessment completed which is taken to a panel. From that panel a decision will be made and hours will be allocated. We will visit you at home at an arranged time to discuss how we can provide these hours to benefit your child and family. If nights are preferable we can either offer overnight respite at your home, or in Swanbourne Garden’s which is our 4 bedded purpose built building in Edgeley.
