The Urgent Community Response (UCR) service provides rapid assessment and treatment at home for people whose health has suddenly deteriorated, helping them avoid unnecessary hospital visits or admissions.
Delivered by a team of registered nurses, physiotherapists, occupational therapists and support workers, the service aims to see patients within two hours and can provide support and monitoring for up to 72 hours.
About the service
The Urgent Community Response (UCR) service is a multidisciplinary team made up of registered nurses, advanced clinical practitioners, physiotherapists, occupational therapists, and support workers. We work closely with GP practices, mental health services, adult social care, ambulance services, Hospital at Home (Virtual Ward), and other community and voluntary sector organisations to provide joined-up care.
Our aim is to assess and treat people in their usual place of residence within two hours of referral, helping to prevent unnecessary hospital attendances and admissions. Depending on a person’s needs, we can provide up to 72 hours of support, treatment, monitoring and rehabilitation to help them remain safe and independent at home.
Who we support
People eligible for referral to Urgent Community Response service need to be:
- Over 18 and registered with a GP practice in Stockport
- Living in Stockport – whether independently, residential or care home
- Those at risk of hospital attendance or admission
- Medically safe to be treated or cared for in a community setting
- In need of urgent assessment or intervention within two hours – must also be safe to wait for two hours.
How we help
The Urgent Community Response service ensures people are supported in the following situations:
| Reduced mobility or function | sudden change in being able to cope with daily living |
| Fall | when there is no serious injury or fracture, or loss of consciousness |
| End-of-Life Support | when urgent care at home for palliative or end-of-life patients, preventing hospital admissions – working alongside palliative care teams |
| Confusion | increased or new confusion including worsening of dementia |
| Urgent Equipment Provision | when equipment needed to support function and keep people safe |
| Deep Vein Thrombosis (DVT) | providing assessment and treatment in the comfort of their own home, preventing hospital admission |
| Respiratory conditions | exacerbations of acute respiratory conditions or infections, supporting hospital avoidance |
| Heart Failure | exacerbations of conditions, supporting hospital avoidance |
| Urinary Tract Infection (UTI) | new symptoms requiring treatment – for example in frail or older people |
| Wound infections/Cellulitis | New symptoms requiring treatment – for example in frail or older people |
How to refer to the service
- Your GP, health or social care professional can refer you to our service if you are registered with a Stockport GP and aged 18 or over.
- Self-referrals are accepted if you have previously been known to the service.
On referral to the service, a clinical triage will be undertaken over the telephone, and an assessment will be arranged by an appropriate professional within two hours and a treatment plan for the patient will be formulated.
The service is available 365 days a year, 8:00am to 10:00pm (including bank holidays)
