The One Stockport Care Coordination Digital Hub provides a single point of access for health and social care professionals to access urgent neighbourhood services across Stockport, for patients in the community with an urgent need.
Our Clinical Call Navigators support professionals by coordinating referrals and directing patients to the most appropriate service, ensuring the right care is delivered in the right place at the right time.

The service is available to:

- Community healthcare professionals
- Hospital teams
- Social care professionals
- Care home staff
- Other authorised health and care professionals
Important: The Care Coordination Digital Hub telephone line is for professional referrals and advice only. It is not a patient helpline and cannot accept calls from patients, carers or members of the public. Patients and Members of the public should contact their GP Practice, NHS 111 or NHS 999 as appropriate.
