Hospital at Home (sometimes called Virtual Ward) provides people with hospital level care and monitoring in the comfort of their own home.
Through this service, suitable patients can stay at home and receive the same level of care as if they were on a hospital ward. Healthcare professionals will visit the patient at home and/or remotely monitor you using technology that keeps us updated on your condition (like heart rate and blood pressure). We call it “the care of hospital, the comfort of home”.
Who we support
People eligible for referral to Hospital at Home need to be:
- Over 18 and registered with a GP practice in Stockport
- Living in Stockport – whether independently, residential or care home
- Those at risk of hospital attendance or admission
- Medically safe to be treated or cared for in a community setting
How we help
The service offers care to patients in the place they call home. We offer individualised care to prevent hospital admission, through intervention and by facilitating early discharge from hospital with support.
When using the service, patients will be monitored by a team of healthcare professionals and could be given some equipment to monitor symptoms, for example, blood pressure or oxygen levels.
Being admitted to Hospital at Home could help relieve some of the fear that some patients experience when coming to hospital, by trying to avoid hospital admission in the first instance and reducing the length their hospital stay. Additionally, being under the care of Hospital at Home should lead to a quicker recovery as patients will be cared for in familiar surroundings, knowing that they are being supported via a collaborative team of healthcare professionals.

| Deep Vein Thrombosis (DVT) | providing assessment, treatment and monitoring in the comfort of their own home, preventing hospital admission |
How to refer to the service
- Your GP, hospital healthcare professional can refer you to our service if you are registered with a Stockport GP.
Referrals are accepted: 8am – 8pm, 7 days a week.
