Stockport NHS Foundation Trust is committed to improving the patient experience for people with learning disabilities and their carers. 

About Our Service

We provide both hospital and community based services working with adults and children  with a learning disability. We offer a creative approach to health care and hold Person Centred practise as a core to our services. We will work directly with the service user and/or carers to address health issues. 

Our Expertise

We have a wide range of expertise in hospital and in community nursing teams, and access to help and support from our colleagues working in specialist teams for people with learning disabilities in Pennine care and the Local authority.

Our focus

Our focus is on bringing about positive change for this vulnerable client group. The main areas of work have been around:

  • Encouraging the use of the Hospital Passports
  • Looking at the preadmission needs of a person with learning disabilities where their hospital admission is planned
  • Applying reasonable adjustments where required
  • Improving discharge information
  • Training our staff involving users and carers

Swanbourne Gardens;

Swanbourne Gardens is a purpose built, fully adapted respite facility in a residential housing part of Stockport, offering short breaks and overnight stays for children and young people, aged 5 to 16 with severe learning disabilities and complex health needs.
