Welcome to the Community Neuro Rehabilitation Service

We provide coordinated multidisciplinary rehabilitation and management for a wide range of neurological conditions such as brain injury, stroke, multiple sclerosis, motor neurone disease and spinal cord injury.

 Our specialist team includes:

• Admin
• Service Lead
• Physiotherapists
• Occupational Therapists
• Speech and Language Therapists
• Clinical Psychologists
• Nurses
• Therapy Assistants/ Assistant Practitioner

The service provides interventions to address an individuals’ own goals and to support the long-term management of their condition. This may include:

• Assessment and intervention to optimise independence in all aspects of daily living.
• Focused rehabilitation treatment, which is specific to the individual, depending on the nature of their physical, emotional, communication and cognitive needs.
• Advice regarding the management of long term conditions for patients and their carers

The team work closely with local neighbourhood teams, other professionals, family and carers to provide holistic care.

Patients will be assessed and treated in their own homes, clinics or appropriate community facilities.

Ou team Our team are highly qualified professionals with specialist training in neurological rehabilitation including:

• Normal movement analysis
• Cognitive rehabilitation
• Spasticity management
• Splinting
• Swallowing management
• Communication

We work closely with consultants, specialist nurses, social services, the voluntary sector and other services. This means that you are offered the best advice available for managing your condition.

Your consultant, GP, health or social care professional can refer you to the team. You must be registered with a Stockport GP. We see people over 16 years who would require neurological rehabilitation.

When a referral is received, the team will contact to make an assessment appointment.

Following assessmnt, we will set joint goals with yourself, and a personalised rehabilitation plan.

Upon discharge, we will look with yourself at options to support self-management and community integration and participation as appropriate.
