Accessing Your Personal Information

The Data Protection 2018 Act and European General Data Protection Regulations gives you a right to access the information we hold about you on our records - this is known as a Subject Access Request.

  • Your request must be in writing or by completing an  Application for Access to Health Records to the Medico-Legal Department. If a request is made on behalf of another person, the Medico-Legal team will advise what documentation is required to enable them to process the request.
     
  • We will acknowledge receipt of your request and ask for any additional information we may need to help us identify your information. We are required to provide you with the information you have requested within 30 days.
