Accessing Your Personal Information

The Data Protection 2018 Act and the UK General Data Protection Regulations gives you a right to access the information we hold about you on our records - this is known as a Subject Access Request.

• Your request need not be in writing, but it will help us locate the information you require if you completed an  Application for Access to Health Records .  If a request is made on behalf of another person, the Medico-Legal team will advise what documentation is required to enable them to process the request.
   
• We will acknowledge receipt of your request and ask for any additional information we may need to help us identify your information. We are required to provide you with the information you have requested within 30 days.

Your request to access your health information will generally be free of charge.  There are occasions, however, when the Trust must charge a fee and this will be explained to you at the time you make your request.
